Actually thats probably not from a dirty weapon. AK's are notrious for being able to fire even when they are incredibly dirty. the problem with most AK's in foreign markets are they are made of cheap metal and are assembled from parts from all over the middle east and africa. This probably happens with some AK's more than people realize.

faulty ammo will do that. primer not igniting powder causing bullet to stay in barrel and next load hitting stuck bullet causing a blowout. they must have been using bad hand loads.
another cause is an overcharged reload. too much powder.
